• • Pays for corrective eye surgery to qualified applicants
  • • Must be correctable by surgery
    • Doctors must agree to maximum payment set forth in application
    • Must be US citizen
    • Must have been rejected for benefits by a government agency
    • Must be sponsored by a Knight Templar
    • Must use original application (no copies allowed)
